Job Description:
A company is looking for a Nurse Case Manager responsible for
Complex Case Management, Disease Management, and Transitional Case
Management.
Key Responsibilities
Provide transition of care calls and conduct assessments to
identify member needs
Collaborate with interdisciplinary care teams to develop and
monitor individualized care plans
Manage a caseload effectively while ensuring adherence to quality
metrics and documentation requirements
Required Qualifications
Current, unrestricted RN license with Compact State Licensure
3+ years of diverse clinical experience, preferably with acutely
ill members
Knowledge of utilization management, quality improvement, and
discharge planning
Experience with Microsoft Office applications
Currently residing in the Mid-West and able to work with CST and
EST Time Zones
