- Manage and coordinate patient care, ensuring optimal use of
resources to achieve quality outcomes. - Collaborate with
healthcare professionals to facilitate patient discharge plans. -
Conduct assessments to identify patient needs and create
individualized care plans. - Monitor patient progress and adjust
care plans as necessary. - Provide education and support to
patients and their families. - Maintain accurate and timely
documentation of patient information. - Participate in one weekend
shift per month as required.
